Release and Assumption of Risk (“AKA Liability Waiver”)
Required for use of the New Balance Recreation Center, º£½ÇÉçÇø Bound Adventure Center, or Wallace Pool, or participation in any programs sponsored by Campus Recreation.

Uº£½ÇÉçÇø students
Sign your 4-year liability waiver electronically on the º£½ÇÉçÇø Street Portal (look for it in your To Do items). Until you do this your membership here will not be activated and you will be required to purchase a day pass.
Day pass purchasers
– All day pass purchasers must complete the day liability waiver at each visit. Anyone bringing a child and/or children under the age of 18 who they are NOT legally responsible for, must have the legal guardian complete aÌýprior to visiting the facility.

Sign your 4-year liability waiver electronically on the º£½ÇÉçÇø Street Portal (found in Employee Self-Service).Ìý
Adults with memberships
Sign on paper at our facility once every 4 years.

Minors
Minors are defined as anyone aged 0-17 and adults who are unable to legally sign for themselves. Exception: Uº£½ÇÉçÇø students under the age of 18 may sign for themselves. See “Uº£½ÇÉçÇø Students”. The parent or **LEGAL** guardian must sign the minor’s Anyone bringing a child and/or children under the age of 18 who they are NOT legally responsible for, must have the legal guardian complete aÌýprior to visiting the facility.
Photo Release Form
Anyone who is having their picture taken by Campus Recreation or University of º£½ÇÉçÇø staff while in a Campus Recreation facility or while participating in a Campus Recreation / º£½ÇÉçÇø Bound activity must complete and sign this form.
